RELEASE OF THE SEA KING BOARD OF INQUIRY REPORT
By FlyingNews @ 1:11 PM :: 259 Views :: 0 Comments :: Military Aviation, Flying Accidents
Chief of Navy, Vice Admiral Russ Shalders AO, CSC, RAN, today released the Board of Inquiry (BOI) Report into the loss of Navy helicopter ‘SHARK 02’, which crashed on Nias, Indonesia on 2 April 2005.

Nine Navy and Air Force members were killed and two seriously injured in the accident. The flight crew and medical personnel were providing humanitarian aid as part of Operation SUMATRA ASSIST II following the Nias earthquake.

Under the wide-ranging Terms of Reference, the Navy Appointing Authority empowered the BOI to not only examine the factors that directly contributed to this tragic accident, but to also critically examine many other associated areas including operations, flight safety, logistics support and personnel management.

“After a meticulous examination of the evidence presented during the inquiry, the Board concluded that the primary cause of the accident was a failure of the flight control system. A key component of the flight control system was not properly secured during maintenance, which resulted in the pilots losing ability to control the aircraft. This was the result of a series of errors and non-compliances with Maintenance Regulations,” Admiral Shalders said.

Airworthiness Directives; Gulfstream Model GIV-X, GV, and GV-SP Series Airplanes
By FlyingNews @ 1:01 PM :: 253 Views :: 0 Comments :: FAA
Federal Aviation Administration

14 CFR Part 39

[Docket No. FAA-2007-28373; Directorate Identifier 2007-NM-110-AD;
Amendment 39-15104; AD 2007-12-25]

RIN 2120-AA64

Airworthiness Directives; Gulfstream Model GIV-X, GV, and GV-SP Series Airplanes

ACTION: Final rule; request for comments.

SUMMARY: The FAA is adopting a new airworthiness directive (AD) for certain Gulfstream Model GIV-X, GV, and GV-SP series airplanes. This AD requires revising the airplane flight manuals (AFMs) of those airplanes, and doing repetitive functional checks of the forward water drain/supply valves and applicable corrective actions. This AD also provides for optional terminating action for the repetitive functional checks. This AD results from reports of failed forward water drain/ supply valves on numerous airplanes, and reports of ice striking the wing-to-body fairings and engine nose cowls of several airplanes. We are issuing this AD to prevent leakage from failed water drain/supply valves allowing the build-up of ice on the airplane, which could separate and strike the airplane structure aft of the failed valves; become ingested by a propulsion engine; or become a hazard to persons or property on the ground.

DATES: This AD becomes effective July 5, 2007.

The Director of the Federal Register approved the incorporation by reference of certain publications listed in the AD as of July 5, 2007.

We must receive comments on this AD by August 20, 2007.

The improvement will provide greater flexibility to serve more markets with existing aircraft, further enhancing the largest international expansion in Delta’s history. Blended winglets are wing tip devices that reduce drag thanks to improved aerodynamics, resulting in a reduction in fuel usage.

“Blended Winglets create an opportunity for us to improve both our operating and fuel efficiency simultaneously,” said Jim Whitehurst, Delta’s Chief Operating Officer. “We expect to see a minimum of 3.5 percent improvements in our fuel consumption and a minimum of 5 percent in our range thanks to this added technology. Our customers will benefit from Delta’s ability to offer more destinations, and Delta benefits from new efficiencies that will improve our operations and better the environment.”

Airworthiness Directives; Diamond Aircraft Industries Model DA 42 Airplanes
By FlyingNews @ 8:02 AM :: 260 Views :: 0 Comments :: FAA
Airworthiness Directives; Diamond Aircraft Industries Model DA 42 Airplanes
AGENCY: Federal Aviation Administration (FAA), Department of Transportation (DOT).
ACTION: Final rule.
––––––––––––––––––––––––––––––––––
SUMMARY: We are adopting a new airworthiness directive (AD) for the products listed above. This
AD results from mandatory continuing airworthiness information (MCAI) issued by an aviation
authority of another country to identify and correct an unsafe condition on an aviation product. The
MCAI describes the unsafe condition as:

Shortly after an engine change, the aluminium fitting attached to the engine gearbox
holding lines and fittings of the propeller control system was found to be cracked. This
led to a pressure loss in the propeller control system following a control system
malfunction and led to an in-flight engine shutdown.

The broken fitting is part of the engine installation and was initially a steel part. It was
later modified by the engine manufacturer to an aluminium design.

Investigation determined that the area is critical for cracks due to combination of mass,
material and installation torque values.

Diamond Aircraft Industries incorporated with Design Change MÄM 42-184 an
additional bracket into production airplanes to improve the installations and prevent
vibration cracks.

We are issuing this AD to require actions to correct the unsafe condition on these products.

DATES: This AD becomes effective July 19, 2007.
On July 19, 2007, the Director of the Federal Register approved the incorporation by reference of
certain publications listed in this AD.

The first two airplanes were delivered in a special dual paint scheme that featured the Lion Air lion on the vertical stabilizer and the Boeing livery colors on the fuselage.

Boeing launched the 737-900ER program in July 2005 when Lion Air announced an order for 30 of the newest 737 model. Lion Air has ordered 60 737-900ERs.

The 737-900ER, the newest member of the Next-Generation 737 airplane family, increases the capability of the 737 by carrying more passengers and flying farther.

Lion Air operates an all-Boeing fleet and is the largest low-cost airline in Asia, with nearly 1 million passengers per month since the airline began operations in June 2000.

[Docket No. FAA-2007-27708; Directorate Identifier 2007-CE-027-AD;
Amendment 39-15083; AD 2007-12-05]

RIN 2120-AA64

Airworthiness Directives; Diamond Aircraft Industries GmbH Model DA 42 Airplanes

AGENCY: Federal Aviation Administration (FAA), Department of
Transportation (DOT).

ACTION: Final Rule.

SUMMARY: We are adopting a new airworthiness directive (AD) for the products listed above. This AD results from mandatory continuing airworthiness information (MCAI) issued by an aviation authority of another country to identify and correct an unsafe condition on an aviation product. The MCAI describes the unsafe condition as:

From airplanes that have installed the Auxiliary Fuel Tank Optional Design Change (O[Auml]M) No. 42-056, three in-service failures of the auxiliary fuel tank venting system have been reported. These failures have led to the inability to supply the complete auxilliary fuel quantity to the main tanks and the collapse of the auxilliary tank. It is suspected that the vent lines were obstructed either by ice accretion under certain climatic conditions or by blockage of the vent valves because of fuel contaminants.

Undetected malfunctions of the venting system and damaged auxiliary fuel tanks may lead to a lower usable fuel quantity, subsequent fuel starvation and/or fuel spillage into the nacelle.

We are issuing this AD to require actions to correct the unsafe condition on these products.

DATES: This AD becomes effective July 16, 2007.
On July 16, 2007, the Director of the Federal Register approved the incorporation by reference of certain publications listed in this AD.

Bird avoidance job going to the dogs
By FlyingNews @ 7:50 AM :: 367 Views :: 0 Comments :: Military Aviation
Officials from the 459th Air Refueling Wing hired a new member to the wing staff.

What makes this new employee different is the recent wing addition is a four-legged member who answers to the name Jackson. Nearly seven months old, Jackson is the only dog hired in the history of the 459th ARW.

Owned by Maj. Eddie Miller, the 459th ARW Safety chief and a pilot, Jackson, a black tri-Australian shepherd, comes to work once or twice a week to train.

"At this point he can run for about 15 minutes, but when he is fully grown he'll be able to run for an hour to scare the birds off the airfield," Major Miller said.

Jackson works with the active duty airfield manager as part of the base's bird avoidance strike hazard program. Jackson will aid in decreasing bird activity in conjunction with other tactics to rid the birds of flying and nesting around the airfield.

The ultimate goal is for fewer birds to get sucked into aircraft engine intakes. Not only can it potentially be hazardous for the aircraft and passengers on board, but also it can be extremely costly, as even the smallest of birds can strike an engine and destroy it. Common birds who fly and nest near the Andrews Air Force Base airfield are seagulls, buzzards, Canadian geese, and red tail hawks.

Airworthiness Directives; Cessna Model 500, 501, 550, 551, S550, 560, 560XL, and 750 Airplanes

ACTION: Final rule.

SUMMARY: The FAA is superseding an existing airworthiness directive (AD), which applies to certain Cessna Model 500, 550, S550, 560, 560XL, and 750 airplanes. That AD currently requires installing identification sleeves on the wires for the positive and negative terminal studs of the engine and/or auxiliary power unit (APU) fire extinguishing bottles, as applicable, and re-connecting the wires to the correct terminal studs. This new AD retains the requirements of the existing AD; adds airplanes to the applicability; and, for certain airplanes only, requires a review of wiring changes made using the original issue of one service bulletin and corrective actions if necessary. This AD results from a determination that additional airplanes are subject to the unsafe condition described in the existing AD. We are issuing this AD to ensure that the fire extinguishing bottles are activated in the event of an engine or APU fire, and that flammable fluids are not supplied during a fire, which could result in an unextinguished fire in the nacelle or APU.

DATES: This AD becomes effective July 6, 2007.
